In order for a pregnancy to occur, the released egg must be taken up and fertilized in the
fallopian tube, then implanted in the uterus. Infertility occurs when there is a deviation from
this chain of events. In most cases, the deviation is due to one of the three following causes:
(1) a problem with the ovarian function or the egg; (2) a problem with the number or activity
of sperm; or (3) a problem with the fallopian tube. If no problems exist in these areas, a
natural pregnancy can occur through sexual intercourse during the ovulation period based on
the “calendar method” or by using ovulation-inducing drugs. However, if any of the
above-mentioned problems exist, artificial insemination, in-vitro fertilization/embryo transfer
(IVF-ET) or intracytoplasmic sperm injection (ICSI), or other methods may be used.
Artificial insemination, a widely used infertility treatment, is a method in which semen is injected into the uterus to coincide with the date of ovulation. The target market of this treatment is men whose sperm is low in number or not very active. IVF-ET or ICSI and other methods are known as Assisted Reproduction Technologies (ART), and involve the precise maneuvering of eggs and sperm under a microscope. In in-vitro fertilization/embryo transfer, the developed egg is removed from the follicle and fertilized in a petri dish with sperm. Cell division occurs within the fertilized egg to eventually form an embryo, at which point it is returned to the uterus. In ICSI, a single sperm is suctioned from the semen under a microscope and then injected into the egg cytoplasm with a thin pipette for fertilization. As with in-vitro fertilization, the fertilized egg is returned to the uterus once it develops normally. Although the success rate for pregnancy is high compared to artificial insemination, state-of-the-art equipment and a high level of technical skill are required to carry out these procedures precisely and smoothly.
There are often instances in which problems with uterine fibroids and ovarian cyst/endometriosis contribute to infertility, and once these problems are treated, natural pregnancy can occur.
