Features of “Ningen Dock” and Comprehensive Health Examinations in Japan

The purposes of health and medical examinations are to discover problems that may be harmful to the future health of the client, and to provide proposals for health promotion support solutions. Health examinations, which include diagnoses, provide comprehensive assessments regarding the condition of the whole body, while medical examinations focus on a specific disease or organ. In Japan, it is a national policy to make it a person’s duty to have basic health examinations. In addition, thanks to supports provided by corporate and other organizations that emphasize preventive medicine, voluntary-type health examinations, which include medical examinations and provide comprehensive assessments, have developed in response to client needs. Such a system of comprehensive health examinations was started in the U.S. about 40 years ago, but did not spread in Europe or the U.S. partly because the social support for it was not available. In Japan, on the other hand, voluntary-type health examinations in particular have developed into the unique form called “Ningen Dock” (from the concept of a “human dry-dock”), because people in Japan are greatly concerned about early diagnosis and early treatment.

The development of the Ningen Dock system has not been dependent on Japan’s national health care system, the medical insurance for the whole nation, but has evolved as a medical service provided by each medical facility in competition with other such facilities, with client satisfaction as the index of success. This progress has come about with the introduction and quality control of the most advanced examination equipment and examinations, with examination results assessed by medical specialists on the basis of Japan’s high-quality medical care, and with careful client support underpinned by the Japanese culture of hospitality.

Accurate Advice and Care Targeting Examination Results, Backed by Japan’s High-Level Medical Care

Health examinations are not just for identifying disorders and referring clients to hospitals and other medical facilities. If anything abnormal is found, it is necessary to provide the client with the most appropriate advice, by determining whether follow-up observations would be sufficient, or, if medical treatment is required, what kind of medical treatment should be provided and what facility would be appropriate for a particular treatment. In Japan, many facilities make such determinations according to international or Japanese standardized clinical practice guidelines created for the respective diseases. To secure such high-quality care, Japan has established a certification system for specialized Ningen Dock and health examination doctors, making great efforts to cultivate human resources for the practice of preventive medicine. This is a groundbreaking effort that has not been seen anywhere else in the world.

Moreover, for such advice and care, it is essential to provide backup by the medical organizations that can actually perform appropriate guidance and treatment. The ultimate goal of medical examinations is to provide health promotion and early treatment, and it can be said that the overall level of medical care in Japan is high enough to attain these goals. Using the network of health care institutions in Japan, each client can be referred to a medical institution capable of providing the treatment that meets the client’s examination result, which is another aspect of Japan’s excellent health and medical examination system.

Meticulous Health Examinations to Support a Wide Range of Disease Structures and Client Needs

Health examinations provide an occasion to realize preventive medicine. An important mission of health examinations is to provide support to improving risk factors that elevate the risk of outbreak of a malignant disease or a cardiovascular disease at an early stage, before subjective symptoms become apparent. In addition to basic examination items, optional items are added that correspond to a client’s disease risk factors. How to select these items is considered according to the structure of diseases in the country. In Japan, the structure of diseases has changed over a very short period, alongside the Westernization of lifestyles and with the increase in the elderly population, and the content of health examinations has been advancing to keep pace with these changes. As a result, we have built up a wide-ranging medical examination system to cope with various disease structures, and that can respond to various needs

Furthermore, on the basis of health examinations, meticulous educational guidance is provided to match individual living patterns and ways of thinking, and it should be possible to provide this kind of care for international patients as well. In addition, we can also conceive of a time in the future when more appropriate and effective educational guidance could be continuously provided according to a clients cultural background and eating habits, via collaboration with medical facilities in the client’s home country.
